1. EXISTING PLAYERS
Current players (played & registered last year) need only pay their fees on the 3rd February and advise of any change in contact details – no forms to fill out!. The fees this year will be $110/1child, $160/2 Children, $190/3 children. Please Note: If fees are paid on or before the 3rd February a $10 discount will apply.
ONLY IF a medical condition has arisen in the past 12 months that we should be made aware of (since completion of lasts years Medical Forms) we will need to update our records – so would ask you also complete the following form:
2. NEW PLAYERS TO THE CLUB
Unfortunately if you are a new player to the club we have a number of forms that need to be completed for both Metro West and Port District. These completed forms will need to be bought to the club on Registration Night along with payment and some proof of age (Birth Certificate, Student ID etc)
3. NEW PLAYERS REQUIRING A CLEARANCE FROM ANOTHER CLUB
New players to Port District that require a clearance will need to complete all the forms in Section 2 (above) as well as the following MWJFL Clearance Form (below). These completed forms will need to be bought to the club on Registration Night along with payment and some proof of age (Birth Certificate, Student ID etc)
We will have some blank forms available at the club, however we would ask that you try to complete forms in advance so we can streamline the processing of forms and payments. Please also endeavour to have the correct amount of money as in past years we have struggled with correct change.
Credit Card facilities will be available but as we only have one terminal there may be delays – cash is best!
Look forward to seeing you on the 11th February. We will have a free sausage sizzle for all Junior Players as well as some games and coaching clinics with senior players. Later that night is the seniors launch with live music – all welcome to hang around and enjoy the festivities.
